It is essential that you provide the required identification to complete the Form I-9 otherwise, Integrity HR Management, LLC will be unable to employ anyone that cannot provide the required identification listed on the Form 1-9 or that has not completed the entire employee packet; therefore, will not be considered an employee and will not be covered under Worker's Comp.

    Child Support, Garnishments, and Payroll Deductions
    Employees must provide Integrity HR Management, LLC with any child support orders, garnishments, or provide a signed payroll Deduction Authorization Form. Deductions will not begin until proper documentation has been received, and may cause a delay in payroll deductions. It is the employee's responsibility to provide proper documentation and verify that proper deductions have been taken.
